Patient Information


Items to Bring with You

To ensure that your office visit goes as smoothly as possible, please take a few minutes to assemble all the items you will need to have on hand when you arrive. Such preparation will ensure not only that you receive the best care possible but that any associated issues, such as insurance and billing, are handled efficiently and correctly.


Please bring the following:

ALL your medications, prescription and nonprescription. We need to see your medication bottles.

Your health insurance cards and any preauthorization information.

Any medical record information such as most recent lab work, cardiac testing, xrays, cardiac catheterizations or other cardiac procedures or surgeries.

A list of drug allergies.

Question to ask the doctor.


Financial resources (cash, American Express, Visa, Mastercard, and Discover accepted) to cover deductibles, co-payments, coinsurance or past due bills.


Prescriptions

We use Surescripts for e-prescibing to exchange information electronically with your pharmacy. When one of our providers writes a prescription for you it will be electronically sent to your pharmacy. We will need to keep your pharmacy updated in our record in order to ensure that your prescriptions are sent to the correct place. If you change pharmacies or mail order companies, please remember to let us know. If we do not have the correct pharmacy this will cause delays in you receiving your prescriptions.


Patient Portal

You may now access the patient portal to update your address, phone number, insurance information and medical history.

If you have not been given an access code at your most recent visit please call our office to obtain one.

You may also use the patient portal to access your most recent lab results and testing results.

We encourage you to click on the patient portal tab to watch a video about the patient portal and to review frequently asked questions.


Fees and Insurance

We require payment of deductibles and co-pays/co-insurance at the time of service. Medicare assignment is accepted for all our patients. If you have secondary insurance, please provide the information when you sign-in. Please be aware that Medicare does not cover routine testing for screening services. For tests that are not covered, you will be requested to sign a waiver indicating that you accept responsibility for these services.

Please see the home page for financial policy and to view list of insurance companies that we participate with. If you do not see you insurance company on that list please give us a call.

Please call our office if you have any questions in regards to your insurance, to determine if we participate, if you need help determining your co-pays, or if you need to set up a payment plan.


Cancellation Policy

We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24-hour notice. Please extend us this courtesy so that other patients may be seen.
